New Patient Visits Are Scheduled Into Two Appointments

Your FIRST visit will typically run about 1.5 hours and will be a chance for you to talk about your health. This gives one of our doctors an opportunity to gather information specifically about your overall state of health in order to prescribe an effective treatment outline. If you have any recent lab work or test results this is a good time to bring copies to the office.

The SECOND appointment is an opportunity for one of our doctors to give you a lot of information regarding your health, your treatment program, if any additional testing is required, and how often you will need to return to the clinic.

Upon calling the office and setting up an appointment, you will be directed to this web site in order to download the new patient forms in advance. If you are not able to prepare the forms before your first appointment, please arrive at the office 15 minutes early.

Referrals

We refer to specialists or other health care providers when appropriate.

Lab Testing

Specific nutritional, functional, or conventional testing may be recommended so that the cause of your health concern can be addressed scientifically.

Payment Options

We accept cash, checks and credit card payments.

Cancellation Policy

It is the responsibility of the patient to notify the office 24 HOURS PRIOR to a scheduled appointment that cannot be kept. A $50 charge will be made for failed or cancelled appointments without prior notification of 24 hours.

Please try to remember that when an appointment is made it has been reserved for you. If you fail to keep this appointment, it prevents me from treating another patient in need of my services.

Please download and complete the following new patient forms and bring them to our office upon your first visit.

	Registration Form - Registration form needed for a new patient visit.
	Homeopathic Intake - Required for a homeopathic consultation.
	Symptom Survey Form - Review of systems forms that will make sure no conditions are missed in your evaluation

You will need Adobe Acrobat Reader to be able to read and print these forms. Now, most computers already have Adobe Acrobat Reader already installed. If you do not have this software, you can download it for FREE here. Thank you.

If you are a PC user, you can right click on the link and select "save as." You can then save it to your desktop and print out the form.

If you are a Mac user, simply drag the link to the desktop, and it will start downloading immediately. You can them open it up and print it.
